Liver is the main site for NET (neuroendocrine tumors) metastasis. Isolated hepatic NETs are not uncommon and could correspond to primary hepatic NETs (PHNETs), a poorly explored entity. Using whole-exome sequencing and transriptomic profiles compared to pancreatic, small-bowel and lung NET, we showed that PHNETs display a foregut-like molecular profile distinct from other types of NETs, with recurrent molecular alterations, remaining a distinct entity with specific profile. Upon exhaustive work-up to exclude unrecognized primary tumor, PHNETs should thus not be considered metastatic NETs from an unknown primary.
