Arri\u00e8re-plan & Objectifs\u00a0:\u00a0<\/strong>La thrombose r\u00e9cente du syst\u00e8me veineux portal (TVP) non maligne non cirrhotique est une maladie rare. Parmi les facteurs de risque de PVT, la maladie \u00e0 cytom\u00e9galovirus (CMV) est g\u00e9n\u00e9ralement r\u00e9pertori\u00e9e sur la base d'un petit nombre de cas signal\u00e9s. Le but de cette \u00e9tude \u00e9tait de d\u00e9terminer les caract\u00e9ristiques et les r\u00e9sultats de la TVP associ\u00e9e \u00e0 la maladie \u00e0 CMV.<\/p>\r\n

M\u00e9thodes\u00a0:\u00a0<\/strong>Nous avons men\u00e9 une \u00e9tude r\u00e9trospective multicentrique fran\u00e7aise comparant des patients atteints d'une maladie PVT et CMV r\u00e9cente (\"CMV positif\"\u00a0; n = 23) aux patients avec TVP r\u00e9cente pour lesquels le test CMV \u00e9tait n\u00e9gatif (\u00ab CMV n\u00e9gatif \u00bb ; n = 53) ou indisponible (\u00ab CMV inconnu \u00bb ; n = 297).<\/p>\r\n

R\u00e9sultats\u00a0:\u00a0<\/strong>Par rapport aux patients des groupes \u00ab\u00a0CMV n\u00e9gatif\u00a0\u00bb et \u00ab\u00a0CMV inconnu\u00a0\u00bb, les patients des groupes \u00ab\u00a0CMV positif\" \u00e9taient plus jeunes, avaient plus fr\u00e9quemment de la fi\u00e8vre et avaient une fr\u00e9quence cardiaque, un nombre de lymphocytes et des taux s\u00e9riques d'ALAT plus \u00e9lev\u00e9s (p 0,01 pour tous). La pr\u00e9valence de l'immunosuppression ne diff\u00e9rait pas entre les 3 groupes (4 %, 4 % et 6 %, respectivement). L'extension de la TVP \u00e9tait similaire entre les 3 groupes. Treize des 23 patients \u00ab positifs pour le CMV \u00bb pr\u00e9sentaient un autre facteur de risque de thrombose. Outre la maladie \u00e0 CMV, le nombre de facteurs de risque de thrombose \u00e9tait similaire entre les 3 groupes. L'h\u00e9t\u00e9rozygotie pour le variant du g\u00e8ne de la prothrombine G20210A \u00e9tait plus fr\u00e9quente chez les patients \u00ab CMV positif \u00bb (22 %) que dans les groupes \u00ab CMV n\u00e9gatif \u00bb (4 %, p = 0,01) et \u00ab CMV inconnu \u00bb (8 %, p = 0,03). Le taux de recanalisation n'a pas \u00e9t\u00e9 influenc\u00e9 par le statut CMV.<\/p>\r\n

Conclusions\u00a0:\u00a0<\/strong>Chez les patients avec une TVP r\u00e9cente, les caract\u00e9ristiques du syndrome de mononucl\u00e9ose devraient faire suspecter une maladie \u00e0 CMV. La maladie \u00e0 CMV n'influence pas l'extension de la thrombose ni la recanalisation. Plus de la moiti\u00e9 des patients \u00ab\u00a0CMV positifs\u00a0\u00bb pr\u00e9sentent un autre facteur de risque de thrombose, avec un lien particulier avec le variant du g\u00e8ne de la prothrombine G20210A.<\/p>\r\n

R\u00e9sum\u00e9 g\u00e9n\u00e9ral\u00a0:\u00a0<\/strong>Les patients atteints de thrombose du syst\u00e8me veineux portal associ\u00e9e au cytom\u00e9galovirus (CMV) ont une extension et une \u00e9volution de la thrombose similaires \u00e0 celles des patients sans maladie \u00e0 CMV. Abstract<\/h2>\r\n

Background & aims:\u00a0<\/strong>Recent non-malignant non-cirrhotic portal venous system thrombosis (PVT) is a rare condition. Among risk factors for PVT, cytomegalovirus (CMV) disease is usually listed based on a small number of reported cases. The aim of this study was to determine the characteristics and outcomes of PVT associated with CMV disease.<\/p>\r\n

Methods:\u00a0<\/strong>We conducted a French multicenter retrospective study comparing patients with recent PVT and CMV disease (\"CMV positive\"; n = 23) to patients with recent PVT for whom CMV testing was negative (\"CMV negative\"; n = 53) or unavailable (\"CMV unknown\"; n = 297).<\/p>\r\n

Results:\u00a0<\/strong>Compared to patients from the \"CMV negative\" and \"CMV unknown\" groups, patients from the \"CMV positive\" group were younger, more frequently had fever, and had higher heart rate, lymphocyte count and serum ALT levels (p \u22640.01 for all). The prevalence of immunosuppression did not differ between the 3 groups (4%, 4% and 6%, respectively). Extension of PVT was similar between the 3 groups. Thirteen out of 23 \"CMV positive\" patients had another risk factor for thrombosis. Besides CMV disease, the number of risk factors for thrombosis was similar between the 3 groups. Heterozygosity for the prothrombin G20210A gene variant was more frequent in \"CMV positive\" patients (22%) than in the \"CMV negative\" (4%, p = 0.01) and \"CMV unknown\" (8%, p = 0.03) groups. Recanalization rate was not influenced by CMV status.<\/p>\r\n

Conclusions:\u00a0<\/strong>In patients with recent PVT, features of mononucleosis syndrome should raise suspicion of CMV disease. CMV disease does not influence thrombosis extension nor recanalization. More than half of \"CMV positive\" patients have another risk factor for thrombosis, with a particular link to the prothrombin G20210A gene variant.<\/p>\r\n

Lay summary:\u00a0<\/strong>Patients with cytomegalovirus (CMV)-associated portal venous system thrombosis have similar thrombosis extension and evolution as patients without CMV disease.
