Some MV subpopulations augment with\u00a0<span class=\"highlight\">cirrhosis<\/span>\u00a0severity and contribute to portal hypertension. This study aimed at determining if plasma MV\u00a0<span class=\"highlight\">levels<\/span>\u00a0can estimate the presence of hepatic venous pressure gradient (HVPG) \u226510 mm Hg and predict\u00a0<span class=\"highlight\">mortality<\/span>\u00a0in\u00a0<span class=\"highlight\">patients<\/span>\u00a0with advanced chronic liver disease. All\u00a0<span class=\"highlight\">patients<\/span>\u00a0with severe fibrosis or\u00a0<span class=\"highlight\">cirrhosis<\/span>\u00a0undergoing liver catheterization between 2013 and 2015 at two centers were prospectively included. We measured circulating\u00a0<span class=\"highlight\">levels<\/span> of annexin V+, platelet, leukocyte, endothelial, and\u00a0<span class=\"highlight\">hepatocyte<\/span>\u00a0MVs. The test cohort included 139\u00a0<span class=\"highlight\">patients<\/span>.\u00a0<span class=\"highlight\">Hepatocyte<\/span>\u00a0MV\u00a0<span class=\"highlight\">levels<\/span>\u00a0were 4.0-fold and 2.2-fold higher in\u00a0<span class=\"highlight\">patients<\/span>\u00a0with Child-Pugh C than in those with Child-Pugh A or B liver disease, respectively.\u00a0<span class=\"highlight\">Levels<\/span>\u00a0of other MV subpopulations were not influenced by liver disease severity.\u00a0<span class=\"highlight\">Hepatocyte<\/span>\u00a0MV\u00a0<span class=\"highlight\">levels<\/span>\u00a0correlated with HVPG but could not identify\u00a0<span class=\"highlight\">patients<\/span>\u00a0with HVPG \u226510 mm Hg.\u00a0<span class=\"highlight\">Hepatocyte<\/span>\u00a0MV level &gt;65 U\/L predicted 6-month\u00a0<span class=\"highlight\">mortality<\/span>\u00a0independently of Child-Pugh score and of Model for End-Stage Liver Disease (MELD).\u00a0<span class=\"highlight\">Patients<\/span>\u00a0with\u00a0<span class=\"highlight\">hepatocyte<\/span>\u00a0MV\u00a0<span class=\"highlight\">levels<\/span>\u00a0&gt;65 U\/L and MELD &gt;15 had a higher 6-month\u00a0<span class=\"highlight\">mortality<\/span>\u00a0than other\u00a0<span class=\"highlight\">patients<\/span>\u00a0(23% versus 3%; P = 0.001). These findings were confirmed in a validation cohort including 103\u00a0<span class=\"highlight\">patients<\/span>.<\/p>\r\n<h4 style=\"text-align: justify;\">CONCLUSION:<\/h4>\r\n<p style=\"text-align: justify;\">Circulating MV\u00a0<span class=\"highlight\">levels<\/span>\u00a0cannot identify\u00a0<span class=\"highlight\">patients<\/span> with HVPG \u226510 mm Hg; by contrast, <span class=\"highlight\">hepatocyte <\/span>MV <span class=\"highlight\">levels <\/span>strongly <span class=\"highlight\">improve <\/span><span class=\"highlight\">prediction<\/span> of 6-month <span class=\"highlight\">mortality<\/span>\u00a0in\u00a0<span class=\"highlight\">patients<\/span>\u00a0with advanced chronic liver disease; therapies associated with decreased\u00a0<span class=\"highlight\">levels<\/span>\u00a0of circulating\u00a0<span class=\"highlight\">hepatocyte<\/span>\u00a0MV might be attractive strategies in\u00a0<span class=\"highlight\">patients<\/span>\u00a0with severe\u00a0<span class=\"highlight\">cirrhosis<\/span>.
